If your physician does not have admitting privileges, our Medical Director or other attending physician will be happy to oversee your care while you are staying with us. Dr. Fatehchand graduated from Madras Medical College, University of Madras, in Madras, lndia. His residency was completed at Highland Hospital/University of Rochester School of Medicine and Dentistry in Rochester, New York. He is board certified in internal medicine. He is a member of the Fellow of American College of Physicians, and is on the American Board of Palliative and Hospice Care. He is also certified in controlled substance (Subuxone) prescription.
He has over 25 yrs of experience in providing care to guests at nursing homes both acute , long term as well as chronic ventilator supported residents. His professional goal is to compassionately provide efficient and effective primary medical care and service to patients and the community.

Dr. Miller received his medical degree from The Ohio State University and performed his residency at Riverside Methodist Hospital. He is Board Certified in Internal Medicine (2005) and Hospice and Palliative Medicine (2012). He is a member of The Society for Post-Acute and Long Term Care Medicine and has been employed at MedOne Physicians since 2005. Dr. Miller is married and has 5 children and enjoys being part of exceeding expectations at The Laurels of Walden Park.
Dr. Banas is a board certified family physician in Columbus, Ohio. He is currently licensed to practice medicine in Ohio, Michigan, and Pennsylvania. He is affiliated with OhioHealth Riverside Methodist Hospital. Dr. Banas graduated from Ohio State University College of Medicine and completed his residency with the Cleveland Clinic at Fairview Hospital and Fellowship Training: Adolescent Medicine at MetroHealth Medical Center.
His special interests include: Academic Medicine, Adolescent Medicine, Eating Disorders, Men's Health, LGBTQ Healthcare, Diabetes, Preventive Medicine.
Dr. Forgue is director of the Laurels of Walden Park in-house Dialysis Program provided by American Renal Associates. He is a graduate from University of Texas Southwestern Medical School and completed his residency at the University of Michigan Health System. Dr. Forgue is board certified in both Internal Medicine and Nephrology. He is affiliated with Central Ohio Nephrology, Ohio Health, Mt. Carmel, and Licking Memorial hospital.
Dr. Costello, is a practicing Pulmonary Disease Specialist. She graduated from Ohio State University College of Medicine in 1982 and has been in practice for 35 years. Dr. Costello also specializes in Critical Care Medicine and Internal Medicine
